Nation's CareLink representatives will help you create an assessment report that is appropriate for the claimant's situation and your specific policy requirements. Objective Assessments are completed on claimants to assist in eligibility decisions and to enable the Care Coordinator Specialists (CCS) to understand the policyholder's health history and care needs. Claimants can be reassessed periodically in order to reevaluate their ongoing needs and modify their care plan as needed.

Plan of Care Development
In addition to the objective assessment, a Plan of Care can be developed based on a claimant's functional and health status, social situation and individual needs. The Plan of Care outlines the providers, hours of service, care needs and emergency plans to ensure that the claimant remains safe while reaching their optimal functional capabilities. The information for the Plan of Care comes directly from the health assessment, from the claimant and family member and, when necessary, from health provider resources and records.

Care Coordination
Nation's CareLink Care Coordination can dramatically reduce your costs and shorten claim duration. The CCS staff researches, gathers and implements provider resources of the highest quality to meet the care needs of the claimant. Nation's CareLink also negotiates significant discounts on provider rates on behalf of the claimant and the insurer. Care Coordination Specialists monitor and assess the physical status, social situation, and individual needs of claimants on an ongoing basis to ensure that their ADL and cognitive care needs are met in the most cost effective manner. Our Care Coordinator Specialists are all Registered Nurses who have a unique background in geriatrics, long term care, home health care, rehabilitation services, and/or case management experience.

The Minnesota Cognitive Acuity Screen (MCAS), designed by leading scientists and physicians for Nation's CareLink, is more than 98.1% accurate in determining the presence of dementia, including Alzheimer's Disease. The MCAS tests 9 different areas of cognition, including short-term memory, comprehension and judgement to ensure that our Care Coordinator Specialists can develop the most appropriate Plan of Care.
